I understand that if my child takes a leave of absence or discontinues participation from CHAT that I am obligated to pay any outstanding fees owed CHAT and that no refund of monies already paid will be given. I must give at least a 5 business day notice prior to the next billing quarter to the swim team administrator by email. If proper notice isn't given a $50.00 processing fee may be charged.
I also understand that I must notify CHAT in writing or email of any absence or departure from the team and those regular dues will accrue until written notice is given to CHAT. Notification of any absence must go to Todd Etherington, head coach of Cottonwood Heights.
If your swimmer leaves the team and decides to come back, there will be a $30 reinstatement fee added to your account.
I understand if my swimmer does not meet the attendance requirement that they may be moved into a different training group.
