By submitting this enrollment, I agree to the following:
To give my child(ren) approval to participate in any and all camp activities. I also assume all risks and hazards incidental to participation and release Fremont Hills Country Club, its members, Officers, Directors, Agents and Employees from any liability.
That I understand that withholding relevant medical or behavioral information from the Camp Director is grounds for dismissal from camp without a refund.
That I understand that cancellations must be made in writing, electronically is acceptable, at least 14 days prior to the camp session start date to receive a full refund minus a Seventy Five Dollar ($75) administrative cancellation fee per Child. No refunds will be given if cancelled less than 14 days prior to the beginning date of the session.
To give my permission to use my child(ren)’s likeness in photographs/videos for promotion and public purposes for Fremont Hills Country Club, with or without identification of me or my child by name in perpetuity. I will make no monetary or other claim against Fremont Hills Country Club for the above state use(s), if any, of the photograph(s)/video(s). Further, I grant permission to use my or my child(ren)’s picture and/or name for the specific purpose of publication on the website www.fremonthills.com. I understand that I may change or withdraw consent for this use by contacting Fremont Hills Country Club in writing.
The undersigned acknowledges that she or he has read this Authorization, Waiver and Release, fully understands it, is of sound mind, is signing it freely, voluntarily, and without duress or coercion and if the undersigned is a Member or spouse of a member is signing this Waiver and Release on behalf of himself/herself and any Minor Children and any other household resident (e.g., parent of Member). The undersigned and Fremont Hills Country Club agree that if any provision of this Authorization, Waiver and Release is found to be invalid or unenforceable for any reason, such provision shall be severed, and the remainder enforced to give effect to the intent of the parties. The undersigned and Fremont Hills Country Club agree that this Authorization, Waiver and Release may be electronically signed, and that such electronically produced signature shall be of the same legal effect, validity and enforceability as a manually executed signature.
