YMCA Membership Forms
MUS Swimmers for both Lower and Upper School need to turn in their registration forms to their coach or the front desk as soon as possible. If you have a family membership you do NOT need to fill out this form.
You do NOT need to attach a check to the form. The YMCA will collect all the information and bill MUS directly. The balance will be put on your school account. Please email Coach Bryan if you have any questions.
Remember: The first middle school meet is this weekend! The next high school meet will be a dual meet against MHEA on Thursday, November 7.
