Also, I have uploaded an Accident/Incident form for the conference for teams to use at their meets.  It can be found in the Documents drop-down menu.  You can also use this link to go directly to the form. Please print the accident/incident forms and have them avilable in case the form needs to be completed at your meet.   

Also, in the Documents drop-down menu, are two deck entry/scratch forms.  We are asking that teams print and use either form for deck entries.  Please have the coaches fill out the form including birthdate and how the swimmer's name is listed on the team's roster.  Last season I spent several hours trying to merge swimmers after receiving confirmation from the coach that it was the same swimmer.  Hopefully, this will help reduce the number of swimmers that are entered by different names at each meet this season.  The forms can be found here and here.
