All necessary forms to CHS swim team try-out and participation. All forms must be completely filled out, including health insurance information before ENTERING the water. Make sure your forms are entirely completed.

Physicals are required each year. (Palms Medical Group in Lake City is offering free sports physicals for the months of June & July)
