If a swimmer is being transported to a swim practice or function by someone other than the parents, permision form is required. Please use this form
If a swimmer is being transported to a meet alone with an adult other than parent, permission form is required. Please use this form
If a swimmer is staying with an unrelated adult at a swim meet, permission is required. Please use this form.
