To participate in Trident Swim Foundation program, you must complete the TSF questionaire as acurately as possible, read & sign the other TSF documents.

To be able to compete in PSAL with GW or PH High Schools, complete & bring the PSAL Parental Concent form and the PSAL Sport Examination Form to your coaches.

To compete with Trident Swim Club in USA Swimming, fill out & bring the Outreach Athlete form to your coaches.

All documents must be returned to the coaches, & must be readable.
