ALL FORMS must be fully completed (including signed physical) prior to the first practice. Your student may physically deliver the completed/signed physical to the Athletic Office/Coach OR photograph/upload the completed/signed physical within FinalForms. Once your student’s physical is received by the athletic department, it will be put into FinalForms. Make sure your athlete signs the forms as well. Final Forms is maintained and supported by the MHS Athletic Department. |
