1. Complete the form below once per team, per season.
2. If you are affiliated with a MS and HS team fill it out twice, once for each team.
3. If you have edits after completing the form, do not fill out the form again. Email Corey Olson with the changes at [email protected].
4. If you are registering your team after October 1, 2025, please complete the form AND you must email Corey Olson at [email protected] to be added to the team contact/registration list.
