I am aware that TIDE’s team website has a Safe Sport information page with links to various important Safe Sport documents, TIDE team policies, parent consent forms, etc. I acknowledge that I have read over the following Safe Sport documents, understand the expectations, and understand that there may be disciplinary action if I choose not to conduct myself according to these expectations.
