The 2012 forms will not be accepted after August 31, 2012.  The 2013 USA-S athlete, outreach and non-athlete forms are good for the rest of 2012, from September 1, 2012 to December 31, 2012 and all of 2013.  Coach USA-S membership is only good as long as all required certifications are kept current.
