All swimmers/parents/guardians need to read all terms carefully, fill information, initial all pages and sign the documents on page 16, 19 and 20, and send the complete initialed and signed document to club email: [email protected]
Please note that sending page 16/19/20 only is not acceptable and will not considered as completing registration.
