New Member Forms

Upon completion of the 2-week trial, the following requirements must be met before your swimmer can continue to swim with the team: 
 

Complete the online registration process.

Included in the registration process are the following:

1.    Payment of the $110 SCSC Registration Fee. 

2.    Payment of the $76 USA/Midwestern Swimming Registration Fee. 

o   The USA/Midwestern Swimming Registration will be completed online through USA Swimming beginning in August. More information to come.

3.    Completion of both the Parent Code of Conduct & Athlete Code of Conduct

4.    Review New Parent Orientation Presentation and complete acknowledgement form

5.    Provide a copy of your athlete's Government-issued birth certificate or ID to Janet Cho

6.    If you are transferring to SCSC from another USA Swimming team, you also need to complete, sign, and submit an Athlete Transfer Form (Word / PDF). There is a $10 fee required with this form.

These documents, along with your check (if you did not pay by credit card during the registration process), must be turned in at the first practice.
