DC Wave Swim Team is committed to the health and safety of all of our team members. USA Swimming's Safe Sport program helps provide our team and its members with important information and resources to help keep our swimmers and team environment safe for all.
All team members, including coaches and parents, are encouraged to report any behavior that they deem to be unsafe. Coaches especially are considered mandatory reporters, and are required by law to report any information they may have or be aware of concerning abuse of a minor. To report a Safe Sport violation please contact the following individuals:

- Carpool Permission Form - for an unrelated adult to carpool /drive an unrelated athlete locally.
- This form is needed for all Minor Athletes traveling to practices or team functions with an unrelated adult. The form needs to be completed prior to each season.
- Competition Permission Form - for an unrelated adult to drive an unrelated athlete to competition.
- This form is needed for all Minor Athletes traveling to competition with an unrelated adult. The form needs to be completed prior to each season.
- Lodging Permission Form - for an adult athlete to lodge with an unrelated minor athlete.
- This form is needed for all Minor Athletes lodging with an 18 or older Athlete for the purpose of training or competition. The form needs to be completed prior to travel.
