THUNDER FORMS

Below are all the forms needed to join Memphis Thunder.  These forms are also included in your membership packets. 

Southeastern LSC Registration Form - Swimmers who are new to USA Swimming and the SE LSC should fill out this form.  Transferring from another LSC?  Use the form below.

Southeastern LSC Transfer Form - If you are coming to Thunder from another LSC, you'll need to transfer to Southeastern Swimming.
