• Dues for the Ukiah Dolphins Winter Water Polo Season are $TBD
• Dues for the Ukiah Dolphins Summer Water Polo Season are $TBD
• All new and returning athletes must fill out the Dolphins 2018 membership packet and 2018 Pacific Swimming registration forms found here
• All athletes must have a 2018 USA Water Polo Silver or Gold Level Membership. Purchase or renew at www.usawaterpolo.org 
• Club Ukiah Dolphins, ID# 1425
• Zone - Pacific Zone, Zone #6
• Discounted USWP memberships for athletes that are eligible for the free/reduced lunch program are available. Go to http://www.usawaterpolo.org/membership/membership-forms.html and print out the "Shieldy Scholarship Fund Form"
• All tournament costs will be additional, split among players who attend including occasional hotel costs etc.

1. Complete the Ukiah Dolphins and Pacific Swimming Membership Forms.

2. Include a check written to Ukiah Dolphins for TBA.
