MAC Status Change

Members wanting to Withdraw from the club can use this link to the Membership Status Change form  and e-mail it to [email protected]. This form must be submitted by the 20th of the month if you wish to avoid the next billing cycle. 

Withdraw:
Notification by the 20th of the month.
If notification is not received by the 20th of the month, your account will be drafted for the next month.
Reinstatement Fee of $100.00 per child if requested within a 6 month period.

Medical Inactive Status: Provide a doctor's note within 7 days from the date of occurrence.
