We are excited to kick off our Lou Malnati’s frozen deep dish pizza coupon fundraiser to support our Blue Devil Swim Club There are a variety of deep dish varieties to choose from: Cheese, Sausage, Pepperoni and the famous Spinach These pizzas are a real treat and make an excellent holiday gift or easy family dinner.
You purchase coupons that can be redeemed at a Lou Malnati's location of your choice. Redeem when convenient. For swimmer safety, we do not encourage door-to-door sales.

Cost: $16 per coupon (all varieties) and the club retains $6 profit per coupon.

- Orders must be prepaid – Checks only – make checks payable to BLUE DEVIL SWIM CLUB.
- Please include phone number of customer on check.
- Before submitting your order form with payment, please make a copy for your records.
- Fill free to make extra order forms if your orders exceed the form provided.
We are encouraging each swimmer to sell at least 10 pizzas.

Each pizza coupon may be redeemed at any area Lou Malnati’s location for one 9” FROZEN deep dish pizza
(sausage, pepperoni, spinach or cheese). You may redeem any number of coupons at a given time but request calling before redeeming 10+ to ensure stock is available. Coupons expire six months from time of issue. Please allow 2-3
weeks once coupons are requested for payment receipt, processing, print and ship. Not valid on Tastesofchicago.com
