Registration for Returning Gators

This registration is for returning Gator team members who have swam with the Gators for at least 1 season since March 2013.  Please read all of the information listed below.  When completing the registration form you are agreeing to everything listed below, and indicating that you fully understand everything listed.  If you have any questions regarding this information, you should contact Coach Taylor at [email protected].  


Medical Release Waiver & Liability Waiver

Medical Release

I certify that I am the parent or legal guardian for my child(ren). I hereby give my permission for any supervisor, coach or other team administrator associated with the St Cloud YMCA Swim Team to seek and give appropriate medical attention for our child(ren) in the event of accident, injury, illness. I will be responsible for any and all costs associated with any necessary medical attention and/or treatment.

I hereby waive, release and forever discharge St Cloud YMCA Swim Team and associated supervisor, coach or other team administrator from all rights and claims for damages, injury, loss to person or property which may be sustained or occur during participation in St Cloud YMCA Swim Team activities, whether or not damages or loss is due to negligence. I hereby acknowledge that my children is (are) physically fit and capable of participation in all Swim Team activities.

Liability Waiver

By registering my child(ren) with the St Cloud YMCA Swim Team, I agree to participate (or allow my child(ren) and family members to participate) in the St Cloud YMCA Swim Team, and hereby release St Cloud YMCA Swim Team, its directors, officers, agents, coaches, and employees from liability for any injury that might occur to myself (or to my child(ren) and family members) while participating in the St Cloud YMCA Swim Team program, including travel to and from training sessions, swim meets or other scheduled team activities.

I agree to indemnify and hold harmless the above mentioned organizations and/or individuals, their agents and/or employees, against any and all liability for personal injury, including injuries resulting in death to me, my child(ren) and/or other family members, or damage to my property, the property to my child(ren) and/or other family members, or both, while I (or my child(ren) or family members) participating in the St Cloud YMCA Swim Team program.


Fees, Payment Options, and Payment Schedule

Fees
When registering for the swim team, you are responsible for the following fees:

  • USA Registration Fee (One time yearly fee, runs Sept of current year through Dec of the next year)
  • Monthly practice fees (Click here).  
  • Swim Meet Fees (Billed per meet, see details below)

Payment Options
Pay in full payment options for coaching fees are as follows:

Pay by check, due by the first of the month after which you register.  (Example: If you register in June, your full season payment is due July 1).  Make all checks out to the St. Cloud YMCA. Checks can be dropped off in the payment box outside the Aquatics Office at the Y, or sent to:

Partial payments for pay in full registration is NOT allowed.  Anyone sending in partial payment will be billed a 10% administrative fee for the total due for the entire season.    

Monthly payment options for coaching fees are as follows:

Monthly payments must be made through our automated payment system on TeamUnify.  If you register for monthly payments, you are required to put your credit card information on file through the TeamUnify software.  Your athlete's registration will not be approved until this has been completed.  

All monthly auto-draft payments are processed on the first of each month using credit card you put on file with your account.  Checks are NOT accepted under any circumstance for monthly swim team coaching fee payments.  All credit card transactions have an additional charge of 2.95%, plus $.30 for each transaction

Swim Meet Fee payment options:

Swim Meet Fees will be billed to your account based on the month when registration occurs.  Meet fee information will always be included in the notes section when you register your athlete for a meet, as well as in the meet information.  

EXAMPLE: Your athlete registers for Y Sections which takes place on February 4.  Registration for this meet is due on January 15.  Swim meet fees for Y Sections will be put on your account in January, and will be automatically processed through your credit card on the 1st of February.

Payment Schedule
USA Registration Fee: Fee to be applied and paid the first of the month following registration.   

Coaching Fees: Coaching fees will be paid on the first of the month for that month (June coaching fees are paid June 1).  If an athlete registers in May and starts swimming in May, May's coaching fees will be added to the June 1 invoice.  

Swim Meet Fees: The first of the month after registration is taken (see example above).


YMCA Membership Start-Up & Termination Agreement

ALL INFORMATION BELOW RELATES TO THE START UP/TERMINATION OF A YMCA MEMBERSHIP.  THIS POLICY IS NOT THE SWIM TEAM REGISTRATION POLICY.  

YMCA Membership Start-Up Agreement
Athletes do not need to be YMCA members to swim on the team, however to participate in Y meets (October-March) athletes must have a membership.  

If you would like to start a membership, this must be done within 7 days of registration.  If a membership is not started up within 7 days of registration for the season, team fees will automatically be converted to non-member fees.  Any memberships started during the middle of the season will result in member pricing for the next full month.  

You must activate your YMCA membership at the Front Desk at the YMCA.   

Membership Termination Agreement
When the swim season is complete, YMCA membership can be put on hold for $10 per month per membership for up to three month (this would prevent any joiners fees from being assessed upon rejoining).  Any YMCA membership cancelation requires 30 days notice.  A "termination form" must be completed and returned to the front desk to terminate a YMCA membership.  YOU are responsible for the termination of your YMCA membership at the end of the season. 

You must terminate your YMCA membership at the Front Desk at the YMCA.


Agreement to Register for the Full Season

I understand that when my athlete is registered, they are registering for the entire season (high school athletes are the ONLY exception.  Fees will be prorated based on their return/departure for their high school swim season).  Registration on a month to month basis is not an option.  

NOTE: This does NOT apply to high school athletes coming into/out of the season for their high school swim season.  It also does NOT apply to Gator Prep and Gator Transition athletes.


REGISTRATION

NEW: Swim team registration set-up in the computer is now on a perpetual basis.  To end your child's registration, you must do so by completing THIS FORM.  You can complete this form right now if you know exactly when your child will be done swimming for our team, or you can wait and complete this form later.  Understand that your child's registration will be for the entire season, so if you register for the fall AND winter season, even if you fill out the form to end membership in February, your membership will run through March.
