STATUS CHANGE/WITHDRAWAL FORM
Swimmers and their families must notify Dolphin Aquatics via the official STATUS CHANGE/WITHDRAWAL FORM (below), no later than the 21st of the month prior to the month the swimmer wishes to withdraw. For example, if your athlete will not be swimming in April, you would need to submit a withdrawal form no later than March 21. There are NO EXCEPTIONS. The DA Status Change/Withdrawal Form is the only method by which a swimmer can withdraw. Emailing or telling a coach or any other DA employee DOES NOT meet inactivation requirements. If your swimmer is withdrawing for medical reason, see instructions below.
To return to the team within the same swim season, you will be invoiced a $50 reactivation fee. If your swimmer is inactive longer than 6 months or ages out of the previous group, they may be asked to attend a swimmer evaluation to determine group placement. We do not have a pause or hold status.
High School Swimming Leave of Absence (LOA) Policy:
Submit your request via email before the 21st of the month to avoid being billed for the following month (e.g., submit request on or before January 21 to not be billed for February).
If a swimmer decides to take a leave of absence to participate in High School Swimming (Jan/Feb-April) and returns to the team by May, your swimmer will not be charged the reactivation fee. High School Swimming is the only sport subject to this arrangement. If your swimmer returns on or after June 1, you will be subject to the regular status change policy and $50 reactivation fee.
Medical Leave of Absence:
Submit your request via the official STATUS CHANGE/WITHDRAWAL FORM and a copy of the Physician's written order. This cannot be done retroactively. Training fees will be waived for any illness or injury if the swimmer is absent for one to three calendar months. (Physician written verification required for Medical LOA.). We cannot hold the swimmers' spot in a specific group, however, should the group you desire, they will be given priority on the waitlist when they are ready to return.
