Required Consent Forms

To ensure the safety and well-being of our athletes and to remain compliant with USA Swimming, Virginia Swimming & Safe Sport requirements, H2okie Aquatics requires parents or legal guardians to complete specific consent forms for activities involving minor athletes. All forms listed below apply to adult-with-minor participation and must be completed prior to the applicable activity.

Parents or legal guardians may be asked to complete one or more of the following:

Local Travel
Authorizes a minor athlete to participate in team activities involving local transportation (e.g., practices, clinics, or events within the local area).

Meet Travel
Provides permission for a minor athlete to travel to swim meets or competitions with the team under approved supervision.

Overnight Lodging
Required for any team travel involving overnight stays, including hotels or other lodging accommodations, and outlines supervision and housing expectations in accordance with Safe Sport policies.
