The Member Services Department of USA Swimming requires a Report of Occurrence (ROO) to be submitted online any time there is an accident or injury during such USA Swimming activities as Sanctioned or Approved swim meets, swimming practices, contracted Swim-a-Thons or approved social events.  The online form does require that certain sections, indicated by an asterisk, be completed before it can be submitted.

Once the submitter has sent the form to USA Swimming AND received the copy response of the report from USA Swimming, it should then be forwarded via e-mail to Dr. Melissa Millerick-May, Michigan Swimming Operational Risk Chair, at [email protected].
