All athletes participating as members of the Ohio All-Star Zone Team must digitally complete the Athlete Code of Conduct, Permission Form, and Medical Form.  These three forms are completed when the account is setup on this website.  Beginning in 2013, there are no more "mail-in" forms to submit.
