Swim Team Camp registration form

Hello Everyone,

We will again be offering the swim team the opportunity to attend Camp Uni together, August 20-22, 2012.

To make this happen we need to have parents volunteer to chaperone.  Chaperones would stay in the cabins with the campers and supervise them during camp activities where needed.  We will need four female chaperones and two male chaperones. 

I would ask that those parents who can chaperone contact me ASAP to get signed up.  I will even take a couple extra if more are interested.  To have the camp we must have the volunteer chaperones.

There will be a bus to take the kids to and from the camp originating from the WS Y. and also returning to the WS Y.  9;30am departure on Monday, August 20, and 2:00pm return on Wednesday, August 22.

I will be a the camp for part of the time. But not for the entire time.

Registration form is attached and under the documents "Odds & ends".  You may register by turning this form in with payment or credit card info to any Y front desk.  I would also ask that everyone register ASAP so we have an idea of how many kids are interested in attending.
