Completed online with registration:

  • Membership Agreement
  • Consent to Electronic Transmission
  • Swimmer Code of Conduct
  • Parent Code of Conduct
  • COVID-19 Liability Release Agreement​

Hard copy submitted:

  • Release and Waiver of Liability and Indemnity Agreement
  • Swimmer Emergency Card
  • Jobs Deposit Check (held until end of season)
  • Fundraising Deposit Check (held until end of season)

***ALL DOCUMENTS (ALONG WITH CHECKS FOR JOBS DEPOSIT, and FUNDRAISING DEPOSIT) MUST BE DROPPED OFF OR MAILED TO:
