PERMISSION FOR AN UNRELATED ADULT PARTICIPANT TO TRAVEL TO COMPETITION ALONE WITH MINOR ATHLETE

permission, and grant an exception to the Minor Athlete Abuse Prevention Policy for                                                                 (minor athlete), to
