Form 'C' for Missouri High School Swimmers 2024-2025

Form 'C' for Kansas High School Swimmers 2024-2025 (Updated 11/27/23 to include all high schools)

This form should be submitted by club coaches to the person listed below PRIOR TO the start of the COMO, conference, or state meet. USA Swimming coaches should include full name, USA ID, and high school on form C of any swimmer who is participating in the observed high school state or conference championship. Please note that all athletes MUST be registered and in good standing in order to have their times uploaded.

For KSHSAA State Meet and all conference meets in the geographic boundaries of Missouri Valley, send to Greg Huskey.
