If a club experiences a personal injury or property damage (and requires filing a report with MVS), use this link to submit the necessary information to USA Swimming. The Report of Occurence form must be submitted any time there is an accident of injury during such activities as sanctioned or approved meets, swimming practices, contracted Swim-a-Thons or approved social events. After the form is submitted, the submitter will receive an email confirmation. Referee's Meet Report

This form is used to report the overall conduct of an MVS sanctioned meet. Copies of the report are automatically forwarded to the Officials Chair, Sanctions Chair and the General Chair.

MVS Official's Name Tag Order Form

Hasty Awards has developed a name tag that can be used by all MVS officials. The tags are 3-1/4" long by 1-1/4" high with blue engraving on a white background. 

Maxwell Excellence Award Nomination Form

The USA Swimming Officials Committee, Swimming World Magazine and Maxwell Awards has initiated a program to recognize local officials for their efforts and service to swimming. Use this form to nominate anyone you feel has made a positive contribution to the LSC competitive program in the current season.

Male and Female Volunteer of the Year Award / Meet Official of the Year Award

Use this form to submit nominations for male and female Volunteer of the Year awards.
