Midlakes Safety Concern Reporting Process
If you experienced or witnessed an incident that gave you reason to believe that there is a safety concern regarding physical or sexual misconduct by individuals connected with a Midlakes team, please immediately report your concern so it can be addressed. Below is the reporting pathway for how to report your concern.
- Notify the club where the incident occurred. Speak to the club manager, coach, and parent rep for the Midlakes team. The club personnel will investigate the incident, attempt to resolve it, and will also report it to Midlakes.
- Notify the Midlakes Safety Training Administrator at [email protected]. The Safety Training Administrator will be the point of contact for reporting and will escalate to the Midlakes executive team who will assemble a small committee for tracking and investigation of the issue. Midlakes will help determine if there is jurisdiction by Midlakes, i.e. whether it is a club issue or a Midlakes team issue, and help the club and those affected by the incident come to a resolution.
- Notify the U.S. Center for SafeSport via their online reporting system or phone call: Report a Concern | U.S. Center for SafeSport. SafeSport will begin an incident investigation and determine if they have jurisdiction, i.e. are the people involved in the incident members of a national governing body such as USA Swimming, U.S. Soccer Federation, et al. If SafeSport determines they have jurisdiction, then they will assist in bringing the incident to a resolution, and if not then they will refer the incident back to the Midlakes League.
If the incident did not involve participants connected with a Midlakes team or did not occur during a Midlakes event, then you only need to contact the club personnel since Midlakes and SafeSport would not have any jurisdiction.
If the safety concern is only related to property or equipment at a Midlakes club location, then notify that club first. If you do not receive satisfactory resolution of your concern, you may report your concern to the Midlakes President.
