Guest Waiver Form

This form should be printed and filled out prior to the guest coming to the pool with the member.  Each guest is limited to 2 visits per year regardless if invited/sponsored by different members on different occasions.  

11-13 Parent Authorization Form

This is the form member parents fill out to allow their 11-13 year to come to the pool without parental supervision.  Each child in this age group needs to have one one of these forms on file at the office.
