The coaches will provide this form to the athletes electronically at the Athlete-Parent meeting in October.  If you miss the meeting, please reach out to the coaches.

Completion of a Form and Submission of a Physical via DASD's FamilyID website

A form and a physical (Section 7) must be submitted - and approved by DASD - via DASD’s FamilyID website prior to tryouts – no exceptions.  If the athlete participated in a Fall sport or had a recent physical, you only need to submit the re-certification form (Section 8) via the FamilyID website.  Please defer to the FamilyID website for all forms and dates.

Registration & Payment of Team Dues via TeamUnify

After the roster is finalized, the Boosters will send information about how to register for the team via TeamUnify.  As part of that process, you will need to add a credit card to TeamUnify and you will be assessed team dues via TeamUnify.
